Case Manager · Columbus, GA
Responsible and accountable for clinical management of inpatients and outpatients in specific case groups for an
episode of illness. Integrates nursing process (assess, plan, implement and evaluate) and management process (plan,
organize, direct and control) to provide framework for decision making within each role. Possess’ knowledge
necessary to apply criteria to the medical record with respect to patient’s needs for medical and health care.
Knowledge of levels of care, treatment modalities, and the present health care delivery system, both hospital and
community based.
Essential Job Duties
1.
Assess’ the patient’s physical, mental and psychosocial status, identifying existing or potential needs.
2.
Collaborates with other health care professionals regarding Plan of Care, variances in plan implementation,
achieved outcomes or expected outcomes.
3.
Monitors and evaluates short and long term patient responses to therapeutic interventions and analyzes
patterns of variance from clinical information and outcomes.
4.
Guides Practice Groups in the content development of critical paths, which will yield ongoing patient
specific and aggregate data.
