Sup Clinical Nurse Case Mgr · Houston, TX
This position is responsible for the coordination of all systems/services required for an organized, multidisciplinary team approach and assures quality, cost effective care for the identified patient population.
Position requires: lifting/carrying up to 20 lbs., pushing/pulling up to 60 lbs., bending, stooping, climbing, walking, standing, sitting, and exposure to toxic/hazardous conditions.
Qualifications
Associate Degree or diploma required, BSN preferred. Current valid Registered Nurse License in the State of Texas. Basic Life Support Certification required. Case Management Certification desirable (CCM, CRRN, etc.)
Five (5) years experience as a Registered Nurse in the Clinical area assigned. One - two (1-2) years experience with Case Management, Quality Management, Coding, or Discharge Planning preferred.
Two (2) years management/supervisory experience preferred.
Must have basic level computer application experience/skills (Excel, Word, etc.)
