We have a full time and 2 per diem positions open, full time is Monday through Friday.

The Case Manager will help with the disposition of pts, make connections with community resources and evaluate for appropriate admission criteria.

Must be able to interact with physicians and staff in a corteous and professional manner.

Medicare and insurance knowledge is very helpful.

Current RN license required and prior Case Management experience preferred.
